Master Your Pharmacist Interview

The PPCP is the modern clinical framework pharmacy schools and ASHP-accredited residencies use as the structural backbone of clinical practice. Use it for any clinical-scenario question. Collect: subjective and objective data — chief complaint, current meds and adherence, allergies, relevant labs (SCr, LFTs, INR, vanco trough, cultures), social history, insurance constraints. Assess: identify drug-related problems using the standard taxonomy — indication without therapy, therapy without indication, ineffective drug, dose too low or high, ADR, drug interaction, non-adherence. Plan: a specific recommendation — drug, dose, route, frequency, duration, monitoring plan, and explicit follow-up criteria. Implement: how you carried it out — SBAR-style call to the prescriber, EHR documentation, patient counseling, coordination with nursing and case management. Follow-up: what you'd monitor (efficacy and safety markers), when, and what you'd escalate. Use PPCP language explicitly in clinical scenarios — interviewers note it. Even community and ambulatory candidates should anchor MTM scenarios in PPCP.

Staff pharmacist (hospital or community): HR/recruiter screen (15-30 min, scope, license/MPJE status, schedule, salary, why this site) → pharmacy manager or PIC (30-60 min, mostly behavioral, often with a verification or clinical case scenario) → onsite or panel (60-120 min, 2-5 interviewers including manager, lead pharmacist, technician supervisor, sometimes a P&T committee member, increasingly a clinical specialist). Some health systems add a brief in-person workflow shadow.

PGY1 residency: ERAS-equivalent application via PhORCAS (October-December), ASHP Midyear Clinical Meeting (early December — Personal Placement Service / "showcase" with hundreds of programs in two days), formal interview invitations (December-January), onsite interview day (4-7 hours, mix of program director, residency program coordinator, preceptors across required and elective rotations, current PGY1 and PGY2 residents, sometimes a department director). Most programs include a clinical case presentation or topic discussion the applicant prepares in advance.

PGY2 specialty residency: typically interviewed during a PGY1 year — same Midyear → showcase → onsite arc, with deeper specialty-specific clinical questioning (oncology candidates discuss regimen-specific knowledge, ID candidates cover antibiotic stewardship, critical care covers vasopressor and ICU pharmacotherapy).

Match Day: ASHP's Resident Matching Program. Phase I results in early March; Phase II for unmatched applicants follows.

Managed care, industry, MSL, FDA: longer cycles — recruiter screen, hiring manager phone, panel onsite (often with a case presentation on a managed-care or HEOR topic, or a clinical/scientific deck for MSL roles).
